if you're using cpanel make sure your account name is followed by your database settings.
e.g. your cpanel account on your host is turtles09
your database name is site
your database username is cookies
your database name would be turtles09_site
your database username would be turtles09_cookies
your password would be what you set it as and the host would either be
a) localhost
or
b) the host they provide you with
these settings do apply to windows hosts too (some). i've used wamp on my servers as well as home computer and i know you do not need to enter db info that way.
good to know it's apparently resolved though.
also, those broken image links, it could be that the web server capacity is high and it's loading slow the image isn't there or there's no read permissions, personally even though it's dangerous unless you protect it i chmod everything to 777
